We work in conjunction with your primary care provider (PCP). Ask your PCP (doctor, NP or PA) to fax us notes of your most recent medical visit and a referral/request for services to us at 508-420-7460.
For many insurances we do not need a physician’s referral, but we suggest you inform your PCP if you seek mental health services, and we will always need your medical history and notes before we can offer counseling or neuropsychological (memory) testing. We offer:
